Appointment Policy

Call the office to schedule an appointment. Please arrive 15 minutes early for your first appointment to allow time to complete the medical history forms. We have a 24 hour cancellation and no show policy. Patients who cancel or no-show within 24 hours of their appointment will be assessed a $25 charge. This policy enables better access to the doctor so that you too can be seen on short notice when needed.

Prescription Policy

Our prescription refill policy shares responsiblity for RX refills between the patient and doctor. The doctor will provide enough refills to last until your next scheduled visit. You should make an appointment when you are near the end of your refills. Patients who habitually request refills over the phone may be charged a $25 fee for each request. This policy insures that patients get the proper follow up care they need and it helps you to call the office with less chance of a busy signal. It also enables us to spend our time focusing on patient care as a doctor and not acting as a pharmacist authorizing prescription refills.

Payment for Services

We accept cash, personal check ($25 returned check fee), and VISA, MasterCard and American Express credit cards. Co-pays are expected the day of the appointment and cannot be billed later.

Insurance Plans

We accept Medicare and most commercial insurance plans. Please refer to your insurance company's list of participating physicians to confirm we are an in-network provider. At this time we are unable to accept new patients with Medicaid, Amerigroup, Caresource, Molina or Buckeye insurance plans.
